The Cooperative is a national collective of passionate like-minded people tasked with driving a collaborative approach to the design and implementation of Advance Care Planning in New Zealand.


All people in New Zealand will have access to comprehensive, structured and effective Advance Care Planning.

The films and eLearning resources included in this website were developed by the Northern Region Alliance Limited in partnership with the Cooperative. This was made possible with funding from Health Workforce New Zealand.

The Cooperative will work together to develop a common understanding, framework and direction for Advance Care Planning in all areas of health, for our communities.

The Cooperative will work with government and non-governmental agencies, particularly the Ministry of Health, to advance its aims and objectives within the current legal and ethical context in a manner that is coordinated, evidence-based and outcomes focussed.

  1. To fully engage the community (public and private organisations, NGOs, networks and the general public) in the design and implementation of Advance Care Planning.
  2. To develop training for healthcare workers.
  3. To grow and support a group of Advance Care Planning trained clinicians.
  4. To create and control consistency in Advance Care Planning documentation and messaging.
  5. To influence research on issues related to Advance Care Planning.
  6. To develop funding and resource proposals.
  7. To provide input into and impetus for Information Technology integration.

Membership of the Cooperative is free and open to anyone who is or will be involved in the design and/or delivery of Advance Care Planning services in New Zealand.

How the National Advance Care Planning Cooperative was formed

Following the Inaugural International Advance Care Planning Conference in Melbourne in April 2010, the group of New Zealand delegates agreed to get together to discuss the current state of Advance Care Planning in New Zealand and to look for ways that they could work together. This culminated in the first meeting of this group on 30 June 2010. At this meeting it was felt that New Zealand was ready for a collaborative approach and the National Advance Care Planning Cooperative was born.
