The Cooperative is a national collective of passionate like-minded people tasked with driving a collaborative approach to the design and implementation of Advance Care Planning in New Zealand.
All people in New Zealand will have access to comprehensive, structured and effective Advance Care Planning.

in New Zealand.

Following the Inaugural International Advance Care Planning Conference in Melbourne in April 2010, the group of New Zealand delegates agreed to get together to
discuss the current state of Advance Care Planning in New Zealand and to look for ways that they could work together. This culminated in the first meeting of this
